Here’s the current situation: as of July 1, 2022, passengers entering Portugal—including the Azores and Madeira—are no longer required to present proof of a test with a negative result for COVID-19, or to present a COVID-EU digital certificate of vaccination or recovery certificate issued by third countries, accepted or recognized in Portugal.
The exception to this rule applies to passengers arriving on flights originating in China. They will be required to provide a negative test prior to boarding and then will be subject to random testing on arrival at the Madeira airport.
Having said this, remember that situations are always subject to change, so it’s important to stay abreast of any possible developments by periodically checking with reliable sources. The following three links come recommended: the Madeira airport Covid-19 Passenger Information page, the EU travel and health measures for Portugal, and the U.S Bureau of Travel status for Portugal.
Finally, even at the height of the pandemic in Portugal, despite various restrictions such as mask mandates, social distancing in public places, closures, and limitations on movement (such as not being more than a certain distance from your legal residence), it was never a requirement to be vaccinated. The Portuguese population was extremely compliant on their own, and it never became mandatory.
