Health Records and Prescriptions in Portugal

What experiences have US expats had in moving health and prescription records to the Portugal health system (public & private). We are both 78 and have some pre-existing conditions that require periodic monitoring. We want to be sure that we receive the proper monitoring and prescriptions in Portugal. Also any suggestions on prescription medication for the first 180 days in country would be great.
